Liability Waiver and Release
By checking the box, I acknowledge that I am either the legal parent or guardian of the participating child(ren) and I understand and accept that participating in the activities, classes, and programs conducted by Dance Perform Repeat (DPR) involves inherent risks, hazards, and dangers that can result in serious injury or damage to the participant.
I understand and agree that DPR and its staff, instructors, directors, officers, volunteers, and agents are not responsible for any injury (including without limitation, death), damage or loss that may result to the participant or the participant's property while at DPR's premises or during DPR activities, except to the extent and in the amount covered by DPR's liability insurance.
I voluntarily accept and assume all risks of injury, loss, or damage to the participant or the participant's property, whether or not caused by the negligence or other fault of DPR including but not limited to equipment malfunction from whatever cause, or any other fault of DPR.
Additionally, I hereby release and discharge DPR and its staff, instructors, directors, officers, volunteers, and agents from any and all claims, causes of action, damages, or liabilities for injury, loss, or damage to the participant or to the participant's property that may occur as a result of or in connection with the participant's participation in DPR activities, except to the extent and in the amount covered by DPR's liability insurance.
This release includes, without limitation, any personal injury, death, or damage caused by the negligence, action, or inaction of any of the Released Parties.
I have read this Liability Waiver and Release, I fully understand its contents and I voluntarily agree to its terms.
